The true reason for adding D4 is miscommunication. If they knew, they would probably vote for an item that doesn't conflict with Mantle: the D20.

It seems like he was trying to keep it a pleasant surprise for the fanbase, but unfortunately, because players didn't know about this, they choose the D4, an item that outright conflicts Holy Mantle. Thing is, no one could ever imagine that Ed actually also was planning to buff the Lost by adding Holy Mantle as his starting item. The following blog post, Ed explained the reasoning for supporting the playerbase's choice: D4 is, gameplay-wise, an interesting item in itself which provides for unique strategies and unexpected outcomes, and also it fits lore-wise since the Lost is Isaac who "hasnt found himself", so he rolls the D4 to find himself. The true reason lies within this blog post, where Ed made a poll about what buffs should be applied to the Lost. If I'm wrong, please provide a link to Twitter or Tumblr post where he says it, and I'll gladly change my comment accordingly. As far as I know, Ed never said that it was done specifically for players who want a challenge after the buff in AB. While the other comment is certainly a nice justification for why the Lost starts with D4, I don't believe that was ever the reason for adding it.
